Dr. Kotler’s Personal Rhinoplasty Story
I had broken my nose playing soccer in high school, and so it had a bump and was a little crooked. There was a minor breathing problem. The tip was a bit bulbous. I could not have a rhinoplasty done for the usual reasons, time, and money. I had a long education, you know, college, medical school, internship, four-year residency, two years military active duty, one-year fellowship. Finally, when I was a “free man”, I could have it done. Many might tell you mine was an acceptable nose, but acceptable is a term that often pertains to others and not to the owner. You cannot put the decision out to a vote. It is what you want that matters. So, at the age of 38, I decided to have my rhinoplasty.
I traveled to New York City, because I, like you, wanted to “Do it right the first time.” It was performed by my most influential teacher, one of the three world-renowned experts in rhinoplasty I had trained under and studied with, Howard Diamond, MD.

The Experience
In those days, the procedure was commonly done under local anesthesia with sedation, and therefore I remember some of the details. But, no pain. It was an interesting operation because I had some sense, even though I was quite foggy, as to what was going on. I could hear the shaving of the nasal bones to remove the bump and heard the “crack” when the nasal bones were gently positioned into the right place. For a rhinoplasty surgeon, a fascinating experience.
For you non-rhinoplasty surgeons, in case you are wondering about anesthesia, don’t worry. That is not how it’s done today. All of our patients are done under a very light anesthetic rendered totally detached from the operation by the great drug, Propofol, plus the same local anesthetic I had to make it painless, even when you wake up from the anesthetic. Anesthesia is safer than ever with no postoperative nausea and vomiting and results in a very fast recovery.
The next day I felt great. I took advantage of the postoperative rest period to go to the operating room and visit my surgeon. One day a patient, the next day, back to the same OR as a student. It was a great learning experience being a patient because I now better understood what patients might encounter and endure after surgery.
The pain was so minimal. I never took a pain pill, but certainly one was available should I have needed it. I was anxious to see whether or not a “big pain” would arrive, and therefore held off on any pain medicine for 24 hours, and then realized that low and behold, there was not going to be any big pain to arrive. It was a mild ache, but tolerable. I felt good, had good energy, and while my air passages were not very open, I put up with it, as in those days we had no choice. Today, of course, no one has to have any nasal blockage, after nasal surgery because of our invention, The Kotler Nasal Airway. This email is not a plug for that, although I highly recommend it.
Within five days, I looked good. The splint was off. There was some swelling but no bruising. By the sixth day, I was out and about doing just about everything and even attended a play, as I was in New York City. On the ninth day, I went swimming, and on the tenth day, I flew back to Los Angeles. A day later I returned to my practice. I looked a bit swollen, not perfect, but good enough. Within another week or two, there was no evidence of having had the surgery. The closed rhinoplasty’s calling card, no external scar, saved me from having to “explain” a fresh pink incision line. Over the next couple of months, the nose matured, and I was a happy camper.
In life, there are some major decisions. Education: what and where. Marriage decisions. Where to live. But, also some very personal ones, appearance leading the pack. My decision was one of the three best decisions I made. And, that is exactly what I hear from our patients.”
